Diabetes And You

This program begins with a group education class where you can learn more about diabetes and how to properly manage it. After the class, you will have the opportunity to meet one-on-one with a SWC pharmacist for management and care personalized to you and your needs. The SWC pharmacist will work with your primary healthcare provider or diabetes specialist to optimize your care and management. Participating in this program also qualifies you for no copay on certain generic medications that are used to lower your blood sugar.

Medication Review and Consultation

This program will help address your questions and/or concerns regarding your medication(s). If available, pharmacologic and non-pharmacologic alternative therapy options will be provided. Appointments are available to come in and speak to pharmacist one-on-one.

Tobacco Cessation

We know tobacco cessation can be tough and we’re here to help! A SWC pharmacist can work with you to create a tobacco cessation plan to fit your specific needs. Our pharmacist will also be there to guide and support you on your journey to becoming tobacco free. After successful completion of a SWC approved tobacco cessation program, you may be eligible for a monthly discount as a non-tobacco user.

Healthy Habits

The Health Habits program offers tips on how to become a healthier you! You, your friends, coworkers, and/or family members who are covered under the SEHIP LGHIP will receive one-on-one monthly counseling to give you the information necessary reach your health goals. Your personal counselor will provide weight management tools, nutritional tips, exercise, and other healthy habits necessary to maintain a healthy lifestyle.

Pharmacist Driven Disease Management and Education

In addition to the many services offered through the SWC pharmacy, SWC pharmacists can also help with other chronic disease states, such as, hypertension, dyslipidemia, asthma, chronic obstructive pulmonary disease, and many others. You will get information like aspects of the disease and how to properly manage your condition with lifestyle changes, medications, and other therapies. We can also work with your primary healthcare provider in optimizing your care and management.
