Patient Information and Forms

The Wellness Center provides services to SEHIP members and dependents, ages 18 and over, with no copay for physician, physician assistant, or nurse practitioner visits*. Members eligible for these services are insured by SEIB BCBS groups 13000**, 30000, 60913, 61013, 72013 and Southland group 2000.

Pharmacy benefits are available to members and dependents insured by SEIB BCBS groups 13000, 30000, 60913, 61013, 72013, 74603, 74604, and Southland group 2000. In addition, these members can purchase various over-the-counter products at discounted prices.

Mission

The State Wellness Center’s mission is to provide a convenient, economical source of urgent/acute care for SEIB beneficiaries, but not to replace your relationship with your primary care physician. We do not want to fragment your medical care or disrupt your long-standing relationship with your physician.

Appointments

To schedule an appointment, please call our office at 334-263-8470. All patient appointments are scheduled during regular business hours and confirmed two business days prior to your appointment. Please call 24 hours in advance if you cannot make your appointment.

We also accept “walk-ins” and offer same-day appointments for urgent visits during normal office hours.

When scheduling an appointment, please provide us with your name, date of birth, insurance information and the nature of your visit so we can allow sufficient time for your visit. Always bring your insurance cards to each visit.

Emergencies

In the event of an emergency or life-threatening situation, please call 9-1-1 immediately or go to your nearest emergency center. The State Wellness Center is not an emergency treatment facility.

Prescription Refills

We encourage you to secure refills at the time of your office visit. Prescriptions and refills are only handled during office hours. Check your medicine supply often and allow 2 business days for refills.

Lab Test Results

Test results, including biopsies, normally take four to seven business days. You will be contacted by letter or phone with results. If you have not heard from someone within a week regarding your lab results, please call our office and we will be happy to provide them.
